It's definitely not ringworm, or earmites...it looks almost like mange to me.

Whatever it is looks super itchy. I'd suggest putting something on it to soothe the itch at least - polysporin, or coconut oil would be my first choice.

Akram, keeping my fingers crossed that he's ok and that the biopsy doesn't turn up anything to worry about it.

Blood tests came back good just shows inflammation which we were given antibiotics to give him twice a day. We will have to wait until early next week for the biopsy results to come back. His personality has improved a lot in the last 2 days which is good.

The only reason this came to mind was the repeated antibiotic uses.

Fungal keeps coming into my mind because of a past experience. Very different, but the failed antibiotics haunt me.
Willow was very ill, looking like pneumonia. Antibiotics made her better while on them and for a short time after. Then she'd be sick again. I know we did the antibiotics more than twice, it's been a long time ago. The vet finally decided it must be fungal and sent me to a pharmacy almost 50 miles north and the fungal meds weren't cheap. But it was too late, she hadn't gotten the correct medication soon enough. Just hearing about multiple, failed antibiotic use brings back bad memories.

If you have any questions, at all, do not hesitate to ask your vet. Even if it's "only" a gut feeling, you know your cat better than anyone. Remember, there is no stupid question, only the question never asked.

Good news about the bloodwork. So the inflammation is just on the ears then?

Is he eating again, and no more peeing outside of the box?

The inflammation was on the ear. We didn't use a dermatologist just found another vet. He was doing better but the crusty stuff has spread to his other ear and he has peed 3 times in our bed since Thursday morning. Still waiting for biopsy results. He has been doing a lot of hiding he's not a fan of the oral antibiotics he has to take.
