What's up with this cat?

The cat in question is my neighbor's; I occasionally 'hang out' with him/her because they leave their garage open, where it lounges. There seems to be some lumps in various places and it breaths odd. The lumps are behind the head and around its collarbone; initially I thought it might just be fat deposits, but parts of the lumps feel hard. Its breathing makes a sort of ripple effect if you look at its stomach.

I know he/she didn't used to look like this; it was much healthier looking a year ago. Should I talk to my neighbors about this and/or call my local animal shelter?

The cat looks like (s)he's in serious need a of a vet visit. The clumpy look of the coat is an indicator that all is not well health wise. The body shape and posture is very odd. I would mention it to your neighbor, it's their cat and their responsibility to take care of it. Hopefully these changes have come on gradually and they just didn't realize it, rather than neglecting the cat.

Definitely speak with your neighbors about the cat. At the very least, confirm and gender and age of the cat. This is, indeed, a very strange looking animal. I have to wonder if this might be a VERY pregnant female.

I am concerned with the odd appearance around the neck and belly and the apparent muscle wasting of the hind legs. In spite of the bloated belly and swollen neck area, it appears that the lower spine is prominent, indicating undernourishment.

Definitely not the picture of good health.
