Unhappy litter training continues...

We've had our kitten "Eggs" for nearly 3 months, he is about 5 months old now and our first and only cat. He has never used the litter box consistently despite our many many efforts. He has been vet checked and neutered, there is nothing physically wrong with him.
Initially we kept in a small-ish bathroom with 2 litters trays, his bed, scratching post and water bowl, although he used the litter most of the time, every 2/3 days he would pee or poop on the floor, sometimes next to the tray sometimes on the other side of the room. Both litter trays where scooped at least 4 times daily and we came to feed and play with him regularly.
We have tried both hooded and open litter boxes, the boxes are big enough for 2 cats. We have tried several cat litters including "cat attract", he's currently using "world's best cat litter" as it's the one he seems to have taken to most. We have tried deeper litter and shallow litter, it doesn't seem to make a difference. We taped tin foil down on the places he'd peed/pooped on, but he would just go next to the tin foil, nearly half the bathroom floor had foil on it. We clean all his mess' with bio liquid and go over with vinegar to make sure all the smell is gone.
2 weeks ago we decided to try confinement in a smaller area, so we bought a large dog crate, he has been confined with his bed, litter box, scratching post and water. We feed him 3 times daily at regular intervals and let him out for play for half an hour before feeding. But since then he has had 4 accidents and once even pooped on his food dish.
We don't know what else we can do, please help, i'd be grateful for any advice.

I understand why you've confined him for sleeping and when you're not around to watch him, but are you letting him out to play and be with you for periods of time?

yes we let him out to play with him for a few hours a day and take turns supervising him. He has still tried to pee/poop on the floor a few times when i've been watching him.
He is a lovely kitten, extremely affectionate and loves to lick me and be cuddled, but the litter training has become so frustrating.
