Help me with Franki PLS!

I have an 18 y.o. female tabby cat who has recently started acting strangely. First she started to pee outside her litter box (always on a carpet!) and sometimes pooping or puking as well. At first we though it was because we changed her litter but when we switched back she started to pee and poop in both the litter box and on the carpet. We read somewhere that cats sometimes do this to send the message that they can't tolerate the dry food, which made sense because we discovered whole pieces of it in her vomit. So we switched to only wet food but nothing changed. Sometimes we find clots of blood on the carpet as well, which has only happened a few times before. She was spayed when she was a kitten and has never had her period. Could she be suffering from some kind of cat menopause? She had a full vet checkup only a few months ago and everything was clear. They did say she had elevated sugar but it could have been due to the stress of the examination. Could she have diabetes or another kidney problem? Currently she mainly pees on the carpet but will sometimes poop as well. Yesterday there was a grayish-white poop in her litter box, which I've never seen before.
