I think the oldest known cat door is believed to date back to homes and buildings from ancient Egypt. However, this article states the oldest cat door belongs to the Exeter Cathedral, previously known as the Cathedral Church of Saint Peter.

In the medieval period from 1305 to 1467, records indicate a unique partnership between cats and the church. Cats were not only accepted within the church but encouraged to be there, with custors being paid 13 pence per quarter to take care of them. While the cats themselves weren't paid, the custors received money to ensure the felines were well-fed and took care of the church's rodent problem. To aid their hunting, cat flaps, essentially holes carved into doors, were created to provide easy access to areas where rodents lurked. This tradition lives on with the current church cat, Audrey, who uses the same cat flap as her predecessors.
